Partnerships with Federally Qualified Health Centers
We partner with Federally Qualified Health Centers such as Southern California’s DAP Health to improve their colorectal cancer (CRC) screening rates. To do this, we implement our proven step-by-step system that includes:
Data collection and analysis of the center’s CRC screening rates, patient demographics, and clinic workflows
Root cause investigation to understand what’s needed to improve patient outreach and increase screening rates for at-risk individuals
System and process enhancements using Epic, allowing us to distribute essential educational resources and remind patients to get screened
Coordination with Labcorp for the direct mailing of FIT tests with pre-approved messages in multiple languages
Staff training in all new systems, including a pilot process, KPI monitoring, and feedback gathering
We provide expertise in community health advocacy so we can continue to achieve equity in healthcare. By standardizing the approach of CRC screening across DAP Health clinics, we’ve seen improved health outcomes and adherence to screening guidelines.
